Gynecomastia Procedure Options at Aesthetica
Gynecomastia treatment is not one-size-fits-all. The right approach depends on the composition of the tissue involved — whether it is primarily fatty tissue, glandular tissue, or a combination of both — along with your individual anatomy and goals. This is determined during a thorough clinical assessment at consultation.
At Aesthetica Image Centre, the following procedure options are available:
Liposuction
Liposuction removes excess fatty tissue from the chest area using a small cannula. It is performed under sedation or general anaesthesia in a registered day hospital as a day procedure.
Liposuction is most appropriate where excess fatty tissue (rather than firm glandular tissue) is the primary component of the chest fullness. It is not effective at removing true glandular breast tissue, which is firm and located beneath the nipple — a distinction that must be clarified at assessment before any plan is developed.
Most appropriate when- Fatty tissue is the primary or sole component
- Skin quality is good (the skin needs to contract after the fat is removed)
- The chest fullness is soft rather than firm to the touch
- Firm glandular tissue is present beneath the nipple (excision is required for this)
- Significant skin laxity is present alongside the fatty tissue
Surgical Excision (Glandular Tissue Removal)
Surgical excision removes glandular breast tissue through a small incision placed around the nipple. This is the appropriate approach where firm or dense glandular tissue is present — tissue that liposuction alone cannot adequately address.
The incision is placed carefully to minimise visible scarring, and fine sutures are used to support healing. The procedure is performed under sedation or general anaesthesia in a registered day hospital.
Most appropriate when- A firm, rubbery area of tissue is present beneath one or both nipples
- True gynecomastia (glandular tissue growth) has been confirmed at assessment
- The condition has been stable for a meaningful period
- Small incision around the lower edge of the nipple
- Glandular tissue removed under direct vision
- Drainage suture placed where required and removed at follow-up
- Compression vest fitted before you leave
Combination Approach (Liposuction + Excision)
In many cases, gynecomastia involves both fatty tissue and glandular tissue. Where this is the case, a combination of liposuction and surgical excision may be used in the same procedure — liposuction to address the fatty component, and excision to remove the glandular tissue.
This is one of the most common approaches at Aesthetica, as many patients present with both tissue types present in varying amounts. The specific combination used is determined entirely by what the individual assessment finds.
Most appropriate when- Both fatty and glandular tissue are present (most common presentation in adults)
- Assessment confirms mixed tissue composition
Non-Surgical Option — Accent RF
In appropriate cases, non-surgical body contouring using the Accent RF device (by Alma Lasers) may be considered as an alternative to surgery — particularly where fatty tissue rather than glandular tissue is the primary concern.
This is a course of treatment (typically 6–10 fortnightly sessions) using radiofrequency energy to target localised fatty tissue. There is no downtime, no anaesthesia, and no incisions.
Important: Non-surgical liposuction will not be effective if glandular breast tissue is present. For this reason, a consultation with a doctor is required before this treatment is commenced, to confirm the tissue composition and establish whether the non-surgical pathway is appropriate for your individual circumstances.
- Fatty tissue only is present (no glandular component confirmed at assessment)
- BMI is in the healthy range (approximately 18.5–24.9)
- The patient prefers a non-surgical approach and is realistic about what it can achieve
Hormonal Considerations
In some cases, gynecomastia is linked to an underlying hormonal imbalance or to a medication that is influencing hormone levels. Where this is suspected, investigations may be recommended before any surgical or non-surgical treatment is planned.
If gynecomastia is linked to a medication, discussing a change in medication with the prescribing doctor — where clinically appropriate — may be the first step. Where a hormonal cause is identified and treatable, addressing it may reduce or resolve the gynecomastia without the need for surgery.

- Whether any underlying hormonal cause should be investigated first

Recovery Overview
Recovery varies depending on which procedure is performed:
Liposuction or combination approach- Compression vest worn for approximately 4–6 weeks
- Most patients return to light work within 2–3 days
- More strenuous activity generally appropriate from weeks 2–3
- Lymphatic drainage or gentle massage may be recommended
- Follow-up appointments and post-operative visits included in procedure cost
- No downtime — patients return to normal activities the same day
- Mild redness or warmth may be present for a few hours post-treatment
- Results are gradual and develop over the course of treatment
